This document provides instructions for conducting a Routine Data Quality Assessment (RDQA) to evaluate program or project data quality. It outlines a 6-step process: 1) determine the purpose, 2) select levels and sites, 3) identify indicators and data sources, 4) conduct site visits, 5) review outputs and findings, and 6) develop a strengthening plan. The checklist is meant to assess data quality and systems at various levels, from service delivery points to central aggregation sites. Results are summarized using graphs and an action plan is developed to identify weaknesses and areas for improvement.
